This was a bit of a surprise!! "It's an Australian release, and due to the popular Ozzie sex-comedies 'Fantasm' and its sequel 'Fantasm Comes Again' (released in 1976 and 77 respectively), the label Roadshow may have felt the title of US box office hit 'Phantasm' would have caused confusion with local audiences, so in the early 80’s before its premiere it was retitled 'The Never Dead'.
To achieve this in-film they replaced the original red-text Phantasm title card with white text emblazoning “The Never Dead” in groovy font better suited to a surf board. The film also had an R rating (18+), as most horror films did at the time whether gory or tame" The Never Dead – Phantasm Secrets
