John Landis has a cameo in An American Werewolf in London as a stuntman who is knocked through a plate glass window in the big car wreck in Piccadilly Circus. I would put a screenshot, but you can't see him very clearly.

I'm surprised it hasn't been mentioned yet, but Dario Argento plays the killer's hands in just about all his films.

David Cronenberg has a brief appearance as the obstetrician in The Fly and, I know it's a writer cameo, but William Peter Blatty plays the stressed producer in The Exorcist.
