I don't get it

There's no punchline. A 'knock knock' joke has a structure:

Knock Knock
Who's there?
Isabelle
Isabelle who?
Isabelle necessary on a bike


whereas the joke started above doesn't have a finish:

Knock knock
Who's there?
Doctor
Doctor who?
..................................


See what I mean?

It's a silly joke

Maybe, but the set dressing (i.e the pictures on the wall), the costume (1920's flapper dress) and use of a wig (although it seems to be an anachronistic 1960's style) suggests that it is from a film rather than just a modelling portfolio
